Insurance
I am considered an in-network provider for First Choice Health, Kaiser Permanente PPO, and an out-of-network provider for all other insurance companies. As an out-of-network provider I do not bill insurance directly. At the end of our session I can provide you with a receipt containing the information your insurance company will need to determine reimbursement. It is your responsibility to find out whether or not your insurance policy will cover my services.
Contacting your insurance provider is simple. Here are some questions you can ask when you call:
- Does my policy provide mental health benefits?
- What is the amount covered per session?
- Does my deductible apply?
- How many sessions are covered by my plan?
- What is the percentage you cover for out-of-network providers?
- Do I need a referral?
- Does my policy cover couples and family therapy, or only individual counseling?
- Can you provide me with the CPT codes that my insurance covers?
Good Faith Estimate
